Flight Data Analysis (FDA) / Flight Data Monitoring (FDM) / (FOQA) Flight Operation Quality Assurance programs began in the 1960s with two European airlines – British Airways and TAP Air Portugal. During the late 1990s, formal FDA programs became commonplace at major airlines.
